Service Description
Having aches and pain or areas of concern? Or have you had an injury/surgery that is still limiting? This is a formal physical therapy musculoskeletal exam performed by a licensed physical therapist designed for a person with a more involved medical history. This exam will assess ROM, strength, flexibility, mechanics to better help the client achieve their specific goals. *For those inquiring if able to use insurance, please submit copy of insurance card and date of birth to maxperformgo@gmail.com in order to verify coverage. May or may not be reimbursable dependent on insurance plan.
Cancellation Policy
Cancellation Policy: Please cancel your appointment at least 48 hours in advance to avoid a cancellation fee. Late cancellations and "no-shows" will incur a $75 fee. *Exclusions may apply with a physician's note. We appreciate your understanding!
